Job Title:

LPN Visits Nurse

We are seeking a dedicated and compassionate LPN (Licensed Practical Nurse) Visits Nurse to join our dynamic healthcare team. In this role, you will provide essential in-home care services to patients, enhancing their quality of life while ensuring adherence to medical standards and protocols. Key Responsibilities:
  • - Conduct in-home visits to assess patient health and implement care plans as directed by healthcare providers.

  • - Administer medications, injections, and treatments as prescribed.

Qualifications:
  • - Current and valid LPN license in the state of employment.
  • - Proven experience in a clinical or home health setting is preferred.
  • - Strong organizational and time-management skills.
  • - Excellent interpersonal and communication abilities.
  • - Ability to work independently and manage multiple patients efficiently.
  • - Proficiency in using electronic health records (EHR) systems.
  • - Valid driver's license and access to a reliable vehicle for travel to patient homes.
