Registered Nurse; RN - Home Health - per visit

Position

BAYADA Home Health Care is seeking a Registered Nurse (RN) to serve as a Home Health Registered Nurse in Pawcatuck, Connecticut. The role involves providing skilled nursing and rehabilitative care to adult and geriatric patients in their homes across the surrounding towns.

You will be a member of a multi-disciplinary team, delivering compassionate, high-quality care. The position requires independent decision-making in the home setting and collaboration with other team members.

• Use strong head-to-toe assessment skills to identify subtle changes in patient condition.
• Utilize experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care).
• Triage and manage urgent clinical situations independently.
• Perform medication reconciliation, teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.

• Ability to work independently while escalating concerns appropriately.
• Comfortable making real-time clinical decisions without on-site support.
• Strong problem-solving in unpredictable home environments.
• Ability to manage a dynamic visit schedule and prioritize urgent visits.
• Efficient point-of-care documentation and technology use.
• Strong attention to detail while meeting productivity expectations.
• Comfortable providing care in varied home environments.
• Flexible and adaptable to changing patient needs and schedules.
• Demonstrates cultural sensitivity and respect for patient autonomy.

• Current nursing license in good standing in the state of Rhode Island.
• Graduated from an accredited, approved nursing program (transcript or diploma evidence).
• At least one year of recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med/surg, sub-acute care, or rehab units.
• Familiarity with acute and post-acute care conditions and treatments.
