Position Summary:

At Stony Brook Medicine, our Social Workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings.

Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with: mental illness and serious emotional disturbance; mental retardation and developmental disabilities; work-related illnesses or injuries; chemical addiction; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention, habilitation, training, supervision and re-socialization, or confinement.

Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.

Duties of an Outpatient Social Worker II may include the following but are not limited to:
  • Psychosocial assessment of selected cases.
  • Response to positive Social Determinants of Health Screening and Follow-up
  • Response to positive SBIRT screening
  • Extensive work with benefit and entitlement programs
  • Counseling and Crisis Intervention
  • Coverage of multiple clinic settings
  • Work with undomiciled patients and patients with financial challenges
  • Coordinate with SB and other hospital discharge planning team as needed
  • Work with community agencies in regards to transitional care
  • Coordination with insurance companies, insurance verification, advocacy with outside agencies.
  • Assist with access to care and barriers to care including barriers to medication access, food insecurity and transportation
  • Assist patient safety and family concerns
  • Assist patient financial services, benefits/ entitlements or with Medicaid applications as needed.
  • Coordination with CPS, APS, OMH, OPWDD, DSS, and other social service and oversight agencies.
  • Assist patients and families with advanced directives.
  • Supervision of professional social work students.
  • Active involved in performance improvement activities including program development, data collection and analysis of cases and follow up
  • Geriatric and chronic illness case management
  • Long term care planning
  • Coordination with Clinically Integrated Network and partner hospitals
  • Inpatient cross coverage as indicated
  • Use of personal vehicle for work-related travel as needed
Qualifications:

Required Qualifications: MSW from an accredited Social Work Education School. New York State LMSW or LCSW. Three years post Master's. Experience in medical health care setting or medical clinic setting.

Demonstrates excellent written and oral communication skills. Demonstrates strong Psychosocial Assessment skill set. Demonstrates understanding of hospital systems, transition to care issues, social work with different patient populations. Understanding of integrated health care model and Social Determinants of Health. Working knowledge of case management and health homes.

Working knowledge of Long Island and Metropolitan NY Community resources. Working knowledge of all benefit and entitlement programs. Demonstrates patient first, patient advocacy and service excellence. Computer literate and understanding of data analysis. Demonstrates the following: understanding of community systems; demonstrates ability for project management and action planning; demonstrates teamwork and team leader focus; understanding of data collection and statistics. Demonstrates an ability to manage a large and complex case load and an ability to work independently.

Preferred Qualifications: Experience in medical clinics.
