at 11:59 PM Eastern TimeDescriptionStony Brook Medicine’s Nurse Practitioners and Physician Assistants are valuable members of our team who provide clinical services to our patient population. We are seeking candidates to work full-time in our primary care general medicine/geriatric office to provide adult patient care under the supervision of the identified supervising physician. Qualified candidates will demonstrate superior patient care and possess outstanding communication skills while adhering to our high standard of excellence.
Duties may include the following but are not limited to:- Perform urgent care visits, follow-up office visits and transitional care management visits as needed. Document all visits into the electronic medical record in a timely fashion.
- Perform (or learn to perform) comprehensive geriatric assessment, including administration of functional screens, cognitive and depression screens and gait and balance assessments. Perform Annual Medicare Wellness Visits.
- Review laboratory, radiology, pathology, cardiac and other testing results with patients. Refill prescriptions and prescribe new medications as needed. Triage patient messages when necessary. Cover electronic medical records inbox when needed.
- Coordinate follow-up care including consultation referrals, ordering of diagnostic tests, arranging for home care services, ordering durable medical equipment, etc.
- Arrange for skilled nursing facility/subacute rehab, ED transfer and/or inpatient admission for patients when necessary. Note that this is an ambulatory practice and no inpatient work is required.
- Perform relevant outpatient procedures (e.g., cerumen removal) as clinically indicated and as permitted by practice agreement.
- Participate in clinical research programs and quality improvement initiatives within the Division of General Internal Medicine, Geriatrics and Hospital Medicine.
- Perform other duties as assigned by collaborating physician, medical director and/or division chief.
REQUIRED QUALIFICATIONS:
Nurse Practitioner: NYS license and certification as a Nurse Practitioner. Master’s Degree (foreign equivalent or higher) in Nursing or completion of a course for a Nurse Practitioner and certification as a Nurse Practitioner in Adult/Geriatric care. Certification in Family Medicine with experience in the ambulatory setting is also acceptable. One year full‑time working experience in Adult or Geriatric care.
Physician Assistant: NYS license and certification as a Physician Assistant. Bachelor’s degree (foreign equivalent or higher) from an accredited school of Physician Assistants. One year full‑time working experience in Adult or Geriatric care.
PREFERRED QUALIFICATIONS: Additional years of Geriatric or Primary Care Internal Medicine experience, previous experience in ambulatory care. Motivation, dedication and flexibility are all desired qualities.
The selected candidate must successfully clear a background investigation.
