Summary:

The AFC Intake Coordinator will provide great customer service to all those inquiring and applying for Tempus Unlimited Adult Foster Care Program. This role will perform quality data entry of key events and actions. This role will insure swift administrative processes, to maximize all community access to the AFC program by AFC members and AFC Caregivers. This role will support all administrative aspects of the AFC program.

The role will at times, interact with other departments to ensure the best referral and/or intake experience for the caller.

Essential Functions
  • Develop good knowledge of the Mass Health AFC Program.
  • Process all incoming AFC program referrals, and enter the demographics into Zuke, if there seems to be eligibility.
  • Understand Mass Health Fee For Service
  • Understand SCO and One Care Plans, including PACE or CCM
  • Confirm all referrals directly with the AFC Member, and confirm there is an AFC Caregiver, if available.
  • Date record AFC member and AFC Caregiver documents in Zuke and forward to Laserfiche for filing. Record in Zuke as topic Administration in AFC member record.
  • Process all incoming AFC faxes, emails and correspondence, and process accordingly.
  • Log in Zuke the Prior Authorization dates, and send to folder for Laser fiche filing. Track LTSS process and Prior Authorization workflows in Zuke and LTSS Portal, accordingly
  • Support AFC staff to insure AFC member and AFC Caregiver information is correctly entered and filed in Zuke, and Laserfiche.
  • Collect AFC Caregiver logs as priority according to AFC Stipend Payment Schedule. Once Logs are entered, work with AFC Program Manager to approve billing.
  • Assist with response to any questions relating to Zuke data and billing.
  • Review, quality check, and process mileage/expenses for AFC staff.
  • Assist Program Supervisor and Manager with annual, admission, discharge, or other types of surveys.
  • Track AFC Caregiver qualifications and assist with staying ahead of due dates and any annual, quarterly, semi annual deadlines.
  • Send out and assist in processing Primary and Alternate Caregiver documents, and update Zuke accordingly. Maintain AFC Caregiver dates in Zuke, and send those dated documents to Laserfiche for filing.
  • Provide positive phone customer service for all those inquiring about Tempus Unlimited AFC program or any other programs.
  • Fax PCP or any other relevant documents for review and/or signature.
  • Maintain Zuke records by recording all transactions, and sending those dated documents for Laserfiche filing.
  • Assist with processing all AFC program terminations and verify case and Zuke closure
  • Process no contact letters should Care Manager and/or RN have difficulty reaching the consumer.
  • Assist with processing termination, caregiver payment letters and other correspondence as needed
  • Participate in weekly AFC meetings and offer input to maximize program efficiencies
  • Additional assignments as needed

Required Education

Minimum education requirement of a High School Diploma, General Education Development (GED), or High School Equivalency Test (HiSet)

Work Environment

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

  • Must be able to lift 25 pounds
