Our company is looking to hire a Home care admissions nurse.
Responsibilities- Completes an initial home health assessment of patient and family to determine home care needs; provides a full physical assessment and history of current and past illnesses.
- Utilizes the nursing process in planning, providing, and evaluating skilled needs.
- Completes a comprehensive patient assessment reflected in accurate OASIS scoring.
- Develops a care plan with goals based on nursing diagnoses and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes patient and family in planning and revising care.
- Graduate of an accredited school of nursing.
- At least 1 year of recent home Care experience
- Ability to exercise initiative and independent judgment.
