Stoughton Health has an excellent opportunity for an RN Case Manager to join our Medical‑Surgical team on a part‑time basis.
Responsibilities- Review benefits for planned admissions.
- Conduct concurrent chart review for medical acuity.
- Coordinate admissions to supportive care.
- Participate in discharge planning within the multidisciplinary care team.
- Assist in assignment of DRGs.
- Communicate with hospital departments and outside agencies.
Education
Required:
Associate’s Degree in Nursing or experience commensurate with a Bachelor’s degree.
Preferred:
Bachelor’s Degree.
Experience
Required:
Five years acute‑care nursing experience.
Preferred:
Prior related work experience in Patient Review or Case Management.
Licensure
Registered Nurse licensed in good standing by the Board of Licensing and Regulation in the State of Wisconsin.
Computer Competency
Basic computer skills. Microsoft Office Suite: opening a document, changing font, saving, and writing a letter.
Part‑time, 0.5 FTE. Week 1: 24 hours. Week 2: 16 hours. Variable schedule Monday‑Friday, 8:00 a.m. to 4:30 p.m.; no weekends.
