Stoughton Health is seeking an RN Case Manager to join our Medical-Surgical team on a part-time basis.
This role reviews benefits for planned admissions, performs concurrent chart reviews for medical acuity, coordinates admissions to supportive care, and participates in discharge planning with the multidisciplinary care team.
It also helps assign DRGs and maintains contacts with hospital departments and outside agencies to ensure quality care and appropriate reimbursement.
