Position: Wellness Nurse (LPN)

Position: Wellness Nurse (LPN) - No Weekends!

Shift

Schedule:

  • 7:00 AM - 7:00 PM
  • 7:00 PM - 11:00 PM
  • 7:00 PM - 7:00 AM
Job Description
  • Provide planning and delivery of direct and indirect patient care through the nursing process of assessment, planning, prevention, and evaluation.
  • Direct the day to day activities of nursing assistants.
  • Prepare and administer medications as ordered by physician and perform other drug administration.
  • Communicate changes in patient clinical condition with physician, nursing supervisor, and family.
  • Complete required record keeping upon residents admission, discharge, transfer; chart and document per facility policy.
Are you?
  • Customer service orientated.
  • Current LPN license in good standing (Licensed Practical Nurse or RN).
  • Thorough working knowledge of current care standards and regulations.
  • Experienced in hands‑on care of memory‑impaired residents.
  • Can maintain and update effective service plans and supervise care staff.
  • Comprehensive knowledge of medication regulations and laws.
  • Various LPN backgrounds acceptable, including new grad, travel, or long‑term care.
  • Experienced in long‑term care preferred.
  • Excellent verbal and written communication.
  • Committed to interacting professionally and with care toward residents, families, and staff.
Requirements
  • Graduate or future graduate from an accredited nursing program.
  • Hold a current state license as a Licensed Practical Nurse.
  • Excellent verbal and written communication.
  • Charge Nurse experience required.
  • Minimum of 1 year experience as an LPN in a long‑term care setting.
  • Natural problem solver.
  • Professional and caring interaction with residents, families, and staff.
