As a RN MDS Nurse at Hudson Springs Nursing and Rehab, you will play a vital role in delivering high-quality, patient-centered care to our residents. Our mission is to help individuals return home as soon as possible, and we're seeking a qualified and compassionate nurse to join our team.
About the Role- Develop, implement, and evaluate individualized care plans for residents
- Conduct MDS assessments and develop care plans
- Coordinate care with the interdisciplinary team
- Manage patient flow and ensure efficient care delivery
- Communicate with residents, families, and healthcare providers
- Perform quality improvement initiatives
- Adhere to regulatory requirements and company policies
