Job Objective

Coordinates delivery of nursing and social services for adult day participants

Minimum Qualifications

Education:

Graduate of accredited nursing school

Experience:

2-3 years of nursing experience. Experience working with the elderly and those with dementia/Alzheimer's Disease, preferred work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus.

Licensure:
State of Connecticut Nursing License; CPR certification

  • Availability of personal vehicle for work related travel (mileage reimbursed). Travels primarily within Fairfield and New Haven counties.
  • All other related duties as assigned.
Essential Job Functions
  • Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled.
  • Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
Salary

$25.50 - $30.60 per hour
