Home Health Registered Nurse PRN

Become a part of our caring community. As a Center Well Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes.

• Provide skilled nursing care to patients working onsite in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes to physicians and the Clinical Manager.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Oversee the work of Home Health Aides and monitor their documentation completion.
• Use your skills to make an impact in patient care and recovery.

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted registered nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver’s license, auto insurance, and reliable transportation.

• One year nursing experience.
• Home health experience.

$54.00 – $75.00 per visit/unit. Full‑time (40 hours per week) employment may earn a starting base pay of $84,900 – $116,800 per year, depending on geography and experience.

This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.

Because this role is part of Humana’s driver safety program, you must have a valid state driver’s license and maintain personal vehicle liability insurance. Insurance must meet your state’s minimum requirements (minimum $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 property damage or higher).
