In this role, you will provide skilled nursing care to patients in their homes, work alongside the RN case managers and help implement the individualized care plans, coordinate with physicians and interdisciplinary team members, educate patients and families, provide hands on treatments and wound care and help individuals achieve greater independence and improved health outcomes.

Stratford VNA is a local, nonprofit home healthcare agency, licensed by the State of Connecticut and Medicare certified. You may be a great fit if you have:

• A current Connecticut RN license
• At least one year of nursing experience
• Strong assessment, clinical, organizational, and communication skills
• A passion for patient education and empowering individuals to manage their health
• The ability to work independently while collaborating effectively with physicians and interdisciplinary team members
• An interest in community-based healthcare

Home health experience is preferred, and experience with Matrix Care is a plus.
