The Clinical Manager is responsible for overseeing the delivery of clinical services and ensuring the highest standards of patient care throughout the agency. This leadership role provides direct supervision and support to professional clinical staff, promotes regulatory compliance, drives quality improvement initiatives, and fosters a positive, patient-centered culture.
The successful candidate will serve as a resource and mentor to staff, assist with clinical decision‑making, support patient care operations, and help ensure outstanding patient experiences and clinical outcomes.
Key Responsibilities- Provide leadership, guidance, and oversight to professional clinical staff.
- Ensure high-quality, compliant, and patient-centered care delivery.
- Support and mentor clinicians through clinical problem‑solving, education, and professional development.
- Directly supervise assigned staff, including performance evaluations, coaching, and ongoing support.
- Review referrals and coordinate patient assignments to ensure appropriate staffing and caseload management.
- Collaborate with physicians, interdisciplinary team members, and community partners.
- Promote regulatory compliance, quality initiatives, and best practices in home health care.
- Support OASIS documentation quality, outcome improvement, and staff education.
- Participate in quality improvement, utilization review, and performance improvement activities.
- Foster a positive work environment that supports employee engagement, teamwork, and retention.
- Participate in supervisor on‑call rotation.
- Current, unrestricted Registered Nurse (RN) license in the State of Connecticut
- Bachelor's degree in Nursing (BSN) or higher
, or successful completion of the Connecticut Department of Public Health required community health nursing coursework - Minimum two years of home health experience as a Registered Nurse
- Working knowledge of home health regulations, clinical operations, and OASIS
- Valid driver's license and automobile insurance
- Previous supervisory or management experience in home health or a related healthcare setting
