RN MDS nurse

We’re seeking a detail-oriented and collaborative MDS Coordinator to lead our resident assessment and care planning processes. If you're a licensed RN with a passion for accuracy and interdisciplinary teamwork, we’d love to hear from you!

Job Duties:

• Oversee and schedule timely completion of MDS assessments

• Review and certify documentation for accuracy and compliance

• Lead care planning meetings and conduct resident interviews

• Enter and transmit assessment data per regulatory requirements

• Analyze Quality Indicator Reports and track resident changes

• Educate staff on MDS documentation and compliance standards

• Collaborate with the Director of Nursing on assessment-related needs

Qualifications:

• Graduate of an accredited nursing program

• Current RN license

• Strong communication, organizational, and analytical skills

• Experience with electronic medical records and regulatory compliance

• Ability to lift up to 25 lbs and work in a clinical environment
