Concerto Renal Services is an industry leader in sub‑acute hemodialysis within a skilled nursing facility and/or long‑term care facility. We are rapidly expanding and focus on reimagining dialysis care through a more convenient, centralized, continuous care model.

Concerto is seeking a Dialysis Nurse to join its team. The Nurse supervises dialysis care in accordance with policy, procedure, and training, and complies with state and federal regulations. The role assumes a hands‑on role, fosters a team approach to patient care, and collaborates with the hemodialysis healthcare team.

• Provide direct hemodialysis care, perform assessments, administer medications, and educate patients on renal health, treatment, and self‑care.
• Delegate tasks to dialysis technicians, supervise their performance, and ensure compliance with clinical standards and regulations.
• Monitor patient response to dialysis, adjust treatment plans as needed, and maintain accurate documentation of interventions and outcomes.
• Work closely with physicians and the healthcare team to coordinate care, report critical lab results, and ensure continuity of treatment.
• Maintain accurate electronic medical records, ensure timely completion of care plans, and adhere to safety and regulatory protocols.

• Graduate of an accredited school of nursing.
• Current state RN license or compact RN license.
• Minimum of one year of nursing experience in any practice area.
• Minimum of three months of dialysis experience.
• BLS provider certification.

This role requires successful completion of the Ishihara Color Blindness Test. It involves prolonged walking, standing, stooping, and bending, with the ability to lift up to 25 lbs. The position demands strong focus and attention to detail. The work environment includes temperature control, moderate noise levels, and potential exposure to infectious materials.
