What You'll Do- Answers incoming phone calls, gathering and processing patient information from various sources for accurate and comprehensive initial assessment of referrals.
- Obtains necessary referral information to assist with the transition to homecare, including demographics, diagnoses, insurance information, physicians' orders, available caregivers, F2F documentation, etc.
- 1 year of Home Health Scheduling preferred
- Medical Office Experience a plus
- Strong communication and organizational skills
- Ability to work independently
- Valid driver's license and reliable transportation (if applicable)
