The Day Habilitation Floor Lead reports to the Day Habilitation Supervisor. The floor lead directly supports individuals with developmental and other disabilities served through the Habilitation Services and is responsible for implementing individuals’ Day Habilitation Service Plan objectives and documenting their progress. The Floor lead will assume the responsibility of the Day Habilitation Supervisor in their absence. This position will be compensated on an hourly basis.
- Ensure appropriate group ratios (max 1:7 within a group area)
- Ensure staff is actively engaging participants by continuous supervision of multiple group areas.
- Ensure attendance collection upon arrival to the program and all transitions between sessions.
- Ensure incident reports are completed and submitted to Program Director.
- Ensure all clients are being appropriately assisted in personal hygiene tasks according to level of need
- Maintain the day program in a clean and orderly fashion. Assign cleaning tasks. Ensure completion by staff.
- Ensure DHSP objectives are being worked towards daily and documented.
- Train staff on specific DHSP/ISP goals and objectives.
- Ensure individuals are spoken to in a respectful manner at all times.
- Ensure staff and individuals are trained in Human Rights.
- Ensure ongoing communication with Day Hab Supervisor, Service Manager and Program Director.
- Maintain program within fixed monthly budget.
- Implement agency policies and procedures as stated in the agency policy manual.
