ScheduleThis full‑time position is 30 hours per week with flexibility to work either three 10‑hour days or four 7.5‑hour days based on preference. IDT meetings are held every other Thursday at 7:00 a.m. The territory includes Sturgeon Bay and northern Door County, including Washington Island. One holiday per year is required.
On‑call coverage includes either one weekend per month or one weekend every other month, depending on rotation preference. Weekend call shifts run Friday 5 p.m.–9 p.m. and Saturday/Sunday 8 a.m.–5 p.m. or 11 a.m.–8 p.m. Holiday shifts are required once per year and run 8 a.m.–5 p.m. or 11 a.m.–8 p.m.
What You’ll DoAs a Social Worker, you will support patients and families through the end‑of‑life journey as part of an interdisciplinary team.
Responsibilities include:
- Psychosocial assessments, individualized care planning, emotional support, counseling, education, and advocacy.
- Coordinate resources and referrals, assess bereavement needs.
- Participate in care planning and IDT meetings.
- Assist with funeral planning as requested.
- Support Volunteer Services and specialty programs.
- Maintain accurate documentation, uphold the Social Work Code of Ethics.
- Participate in a 24/7 coverage rotation.

Requirements- Bachelor’s in Social Work (BSW) from an institution accredited by the Council on Social Work Education; or bachelor’s degree in psychology, sociology, or other fields related to social work along with one year in a healthcare setting. Master’s of Social Work preferred.
- Certified or licensed as a social worker (CSW, CAPSW, CISW, or LCSW) by the State of Wisconsin required.
- Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
