This is an in-house position, offering the opportunity to be fully integrated within our center’s team while also maintaining strong presence in the community and hospital setting.

• Serve as the vital link between our center and hospital partners
• Help guide patients and families through the next step in their care journey
• Work with a supportive, experienced team that is committed to your success
• Make a meaningful impact while driving admissions with purpose and strategy

• Build and maintain strong relationships with medical professionals, hospitals, and community healthcare partners
• Represent and promote the organization’s mission, philosophy, and services both within hospitals and throughout the community
• Attend facility meetings, calls, and educational in‑services
• Maintain accurate and up‑to‑date referral tracking logs
• Provide timely end‑of‑day reports to department heads

• Valid driver’s license required; travel is a key component of this role
• RN, LPN, or LSW preferred, but not required
• Ability to manage multiple residents and referrals in a fast‑paced environment
