This is a full-time 40-hour per week position. The RN helps triage, coordinate, and administer hospice care for patients, families and caregivers through the utilization of hospice services and resources. The hospice RN is responsible for utilizing hospice and community resources to coordinate and drive services through a plan of care to meet the needs of patients and families.

Essential job functions include demonstrating understanding of the hospice philosophy/principles, managing the plan of care from referral through discharge, completing initial nursing assessments, administering medications/treatments, coordinating patient care with team, family, and physician, completing patient discharges, maintaining accurate and relevant clinical notes, communicating with the medical director and attending physician, participating in interdisciplinary team meetings, providing and maintaining a safe environment for the patient, keeping supervisor informed of potentially problematic patient/family issues, demonstrating excellent communication skills, managing multiple priorities, assuming responsibility for personal growth and development, providing complete and timely nursing reports/handoffs, participating in agency quality improvement program, and meeting individual program expectations and goals.

Qualifications include a minimum of one to two years of experience in health care, experience in a hospice or home health preferred, experience in acute care in an institutional setting preferred, understanding hospice philosophy and issues of death/dying, an associate's degree in nursing, current CPR certification, current Massachusetts licensure as a registered nurse, valid driver's license, and hospice and palliative care nurse certification preferred.
