Responsibilities- Conduct adult or geriatric health assessments, including medical history, diagnosis, treatment planning, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Matrix guidelines and protocols.
- Administer and order point‑of‑care and laboratory tests as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
- Work with the primary care physician on patient education and follow‑up as necessary.
- Provide services at the member’s residence, which can be a home or skilled nursing facility.
- Schedule visits based on availability, with flexibility to work Sunday–Saturday from 8 am to 7 pm.

- Competitive pay: earn income based on the number of assessments completed; monthly average of 20–100 visits can yield $1,800–$11,500 based on a per‑assessment rate of $90–$115, depending on state.

- Master’s degree or equivalent experience and satisfactory completion of NP licensure.
- Current NP licensure in state of practice, with prescription authority or capability to obtain it.
- Board certification by AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- One year of NP experience preferred; new graduates encouraged.
- Valid state driver’s license for the state of residence.
- Automobile insurance covering bodily injury and property damage for the state of residence (minimum limits: $100,000 per person/$300,000 per incident; property damage $50,000).
- Collision and comprehensive deductibles of $1,000 or less.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
