Licensed Practical Nurse (LPN) or Registered Nurse (RN)

Charity Care Home Health specializes in providing personalized care that helps patients thrive. We seek professionals with a strong background in public health, home health care, or hospital settings.

Deliver skilled nursing services in a day services environment or home environment, ensuring adherence to the highest standards of care. The role demands clinical expertise and compassionate patient interaction, complying with agency standards and regulations.

• Provide skilled nursing support for intellectual and developmentally disabled persons attending day support programs.
• Monitor patients during day support hours, manage G‑Tube care, including medication administration and feedings per care plan.
• Document and report any changes in condition to the supervising nurse/agency.
• Collaborate with day support staff and family to ensure safety and quality of care.
• Implement and supervise patient care plans while driving quality improvement initiatives.
• Attend scheduled shifts Monday–Friday, 9:00 a.m.–4:00 p.m.; schedule PRN approximately 3–5 days per week.
• Work weekends on a PRN basis.

Monday–Friday, 9:00 a.m.–4:00 p.m.; PRN, approximately 3–5 days per week. Weekends PRN.

• LPN or RN license in the state of Virginia, with graduation from an accredited nursing program.
• Current CPR certification.
• At least 2 years of experience in home health care, skilled care, or a related field.
• Experience managing G‑Tube care preferred.
• Experience with adults with developmental or intellectual disabilities is a plus.
• Reliable transportation, valid Virginia driver’s license, and car insurance.
• Familiarity with home health equipment and patient care documentation.
• Compassionate approach to patient care with adaptability to individual needs.

$26–$46 per hour.
