Licensed Practical Nurse (LPN) / Registered Nurse (RN)

Charity Care Home Health specializes in personalized nursing care and seeks an LPN with a background in public health, home health care, or hospital settings to provide patient‑focused, one‑on‑one care within our day services program.

In this role you will deliver skilled nursing services in a day services environment, ensuring the highest standards of care and compliance with agency standards and regulations.

• Skilled nursing care: medication management, wound care, respiratory support, G‑Tube care and feedings.
• Patient education: educate patients and their families on care plans and health management.
• Documentation and compliance: maintain accurate records and adhere to all regulatory standards.
• Collaborative care: work closely with RNs, medical staff and families to ensure safe and quality care.

Monday–Friday, 9:00 a.m.–4:00 p.m. – PRN, approximately 3–5 days per week.

• LPN or RN license in Virginia, current CPR certification.
• At least 2 years experience in home health care, skilled care, or a related field.
• Experience with G‑Tube care preferred.
• Experience with adults with developmental or intellectual disabilities is a plus.
• Reliable transportation, valid Virginia driver’s license, car insurance.
• Familiarity with home health equipment and patient care documentation.
• Empathy, compassion, adaptability.
• Compensation: $26-46 per hour.
