Licensed Practical Nurse - Hospice

Overview

Sentara Hospice is hiring a Licensed Practical Nurse (LPN) to provide direct, one‑on‑one care to patients in their homes. The role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high‑quality, compassionate care that improves patient well‑being.

City:
Suffolk, VA

Work Shift:

Third (Nights) – 12:00 AM to 8:30 AM, Monday‑Friday.

Full Time (40 hrs/week)

• Provide direct patient care and support.
• Monitor patient health and vital signs.
• Administer medications and treatments as directed.
• Collaborate with RNs, physicians, and care team members.
• Document patient care and observations accurately.

• Trade School Graduate for Nursing (Required)
• LPN License in state of practice (VA) (Required)
• BLS certification within 90 days (Required)
• 2 years of experience (Required)
• Prior hospice experience (Strongly preferred)

Hourly Range: $22.39–$37.00 per hour, including base pay, incentives, and benefits.
