Lake Prince At Home is seeking a dynamic Registered Nurse (RN) to join our Home Health team as a Case Manager. You will coordinate patient care, document according to reimbursement guidelines, and communicate with patients, families, physicians, and Hospice staff.
Travel to meet patients and families at home within the southside region is expected. The ideal candidate holds a current RN license, at least one year of clinical nursing experience and one year of case management experience;
