Moms In Motion / At Home Your Way is seeking a Service Facilitator (SF) Case Manager to visit client homes locally and assist families in navigating Virginia’s Medicaid Waiver programs. You’ll help clients live safe, healthy, and independent lives while traveling to homes on a daily basis.
No degree is required, but you should have at least 2 years of experience helping individuals with disabilities or the elderly. Strong communication skills and a valid driver’s license are essential.
