Clarius Medical Group, PLLC is excited to announce an opening for a dedicated, full-time Nurse Practitioner specializing in Primary Care (Outpatient) for our advanced practice located in Sugar Land, TX. This is an on-site position designed for individuals passionate about delivering exceptional health care. We emphasize a team-oriented approach, where the focus extends beyond treatment to include patient education and the promotion of preventive care.

• Evaluate and manage adult and geriatric patients across a spectrum of needs, including acute conditions, chronic diseases, and preventive care.
• Close care gaps by ensuring systematic and comprehensive health checks, including Annual Wellness Visits (AWV).
• Coordinate necessary laboratory tests, diagnostic work-ups, and referrals to specialty services.
• Maintain accurate and detailed medical records through proficient use of our Electronic Health Record (EHR) system, eClinical
  
  Works, including documentation of visits, order entry, electronic prescribing, and outreach to patients.
• Support and engage in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and telehealth initiatives as required to enhance patient access and continuity of care.
• Actively educate patients and their families on health maintenance and disease prevention, fostering a culture of health awareness and self-care.
• Ensure the highest standards of quality, safety, and compliance with state and federal regulations.

• Current Nurse Practitioner licensure in Texas (TX APRN/NP) alongside national certification from ANCC or AANP.
• Minimum one year of experience in Primary Care or Internal Medicine is preferred; however, recent graduates with strong clinical rotation experience are encouraged to apply.
• Prior experience with the eClinical
  
  Works software suite is an advantage.
• Demonstrated ability in managing telehealth consultations and executing Annual Wellness Visits (AWVs).
• Strong interpersonal, communication, and team collaboration skills.
• Commitment to a patient-centered approach to care and excellence in healthcare delivery.
