Altus Hospice is seeking Per Diem dedicated Hospice Nurse Practitioners to deliver exceptional patient care in the Sugar Land area. You will evaluate health histories, diagnose conditions, and develop personalized care plans under physician supervision.
Responsibilities include collaborating with patients, families and the interdisciplinary team to ensure holistic care, education, and care coordination. A Master's degree and current NP license in Texas are preferred along with hospice experience.
