As a Home Health Registered Nurse, you’ll provide skilled nursing care that helps patients safely recover and manage their health in the comfort of their own homes.
You’ll- Complete comprehensive patient assessments and develop individualized care plans.
- Manage a designated patient caseload while coordinating care with physicians, therapists, and other healthcare professionals.
- Perform skilled nursing visits and evaluate patient progress.
- Educate patients and caregivers on medications, disease management, treatments, and preventive care.
- Identify changes in patient condition and adjust the plan of care as appropriate.
- Document care accurately and maintain compliance with all regulatory requirements.
- Advocate for patients while promoting independence, safety, and quality outcomes.
Required Qualifications
- Current, unrestricted Texas Registered Nurse (RN) license
- Minimum of one (1) year of nursing experience in an acute care, skilled nursing, or clinical setting
- Excellent assessment, communication, and critical thinking skills
- Reliable transportation, valid driver's license, and current auto insurance
- Willingness to travel throughout the assigned coverage area
- Physical Requirements:
Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support up to 50–100 pounds, including assisting with patient positioning, transfers, and moving equipment.
- Home Health or Hospice experience
- Experience completing OASIS assessments
- Homecare Homebase (HCHB) experience
- Associate or Bachelor's Degree in Nursing
