Unlike our facility-based roles, this position operates in a traditional outpatient clinical setting. We welcome all qualified NP certifications (Family, Adult, Gerontology) to apply. • Perform comprehensive health assessments for adult and geriatric populations, heavily focusing on Annual Wellness Visits (AWVs).
• Drive digital health initiatives including Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and telemedicine consultations.
• Utilize eClinical
  
  Works (EHR), Practice Fusion, Wellsky, etc. for precise documentation, diagnostic ordering, and electronic prescribing.
• Formulate targeted care plans that prioritize closing care gaps and lifestyle modifications.

• Holds an active Texas APRN license and National Certification (ANCC or AANP) in any applicable specialty.
• Comfortable operating independently with a 1099 compensation structure and minimal on-call duties.
