Choice was founded in 2007 by David Jackson, a licensed physical therapist. At the heart of his decision to start the business was his grandfather's struggle with Amyotrophic Lateral Sclerosis (Lou Gehrig's Disease) during his childhood. He was, and continues to be, forever grateful to the nurses, therapist, aides, and other healthcare providers that touched his family during that time. The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate, a proven clinician responsible for helping shepherd the care of the seriously ill. The ideal candidate will have a personality type that is predisposed to thrive in an unstructured environment and be okay with reacting to patient needs while being a supportive team member with excellent ability to communicate with team members.

• Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
• Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care; provides training to other staff as needed.
• Performs face-to-face visits at the patient's place of residence.
• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
• Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
• Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
• Is knowledgeable of hospice criteria.
