About Choice:Choice was founded in 2007 by David Jackson, a licensed physical therapist. The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate, a proven clinician responsible for helping shepherd the care of the seriously ill. The ideal candidate will have a personality type that is predisposed to thrive in an unstructured environment and be okay with reacting to patient needs while being a supportive team member with excellent ability to communicate with team members.
Responsibilities- Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
- Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care; provides training to other staff as needed.
- Performs face-to-face visits at the patient's place of residence.
- Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
- Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
- Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
- Is knowledgeable of hospice criteria.
- Current STATE RN and Advanced Practice Nurse License
