As our Nurse
External Care Coordinator
, you'll become a trusted resource throughout the Charleston community. You'll have the freedom to build professional relationships, educate families, create personalized care solutions, and help seniors remain safe and independent at home.

What You'll Do- Become the face of Comfort Keepers in the community.
- Develop lasting partnerships with physicians, hospitals, rehabilitation centers, hospice providers, assisted living communities, and other referral sources.
- Meet with families to understand their needs and recommend customized care solutions.
- Complete in-home assessments and develop individualized care plans.
- Guide clients through every step of their journey with Comfort Keepers.
- Collaborate with an amazing internal team to ensure every client receives exceptional care.
- Help grow one of the most respected home care organizations in the area.

Qualifications- Active LPN or Registered Nurse (RN) license
- Graduate of an accredited nursing program
- Minimum one year of nursing experience
- Home care experience preferred
- Valid driver's license and reliable transportation
- Strong organizational and computer skills
