Job Summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a job in Summit, New Jersey. This full‑time admissions PT will complete OASIS Starts of Care (SOCs) for adult and geriatric patients in Essex County.

• Make home visits to clients in designated geographic territories and conduct Start of Care (Admissions) visits.
• Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities necessary to promote the client’s health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a touch‑pad tablet.

• Current New Jersey Physical Therapist license.
• At least one year of recent work experience as a Physical Therapist.
• OASIS experience required; HCHB strongly preferred.
• Home Health and OASIS experience is required for this position.

Base pay: $68.00 - $75.00 per point (subject to negotiation).

$10,000 Sign on Bonus for full‑time admissions PTs.
