The Clinical Pharmacist delivers comprehensive, patient-centered pharmaceutical care across a variety of specialty areas, including Behavioral Health, Critical Care, Emergency Medicine, Infectious Disease, Pediatrics/Neonates, Oncology, Inpatient Transitions of Care, Investigational Drug Services, and Opioid Stewardship, and other. Responsibilities include reviewing and optimizing medication therapy, performing medication reconciliation, monitoring therapeutic outcomes, managing adverse drug events, and providing patient and caregiver education. The pharmacist collaborates with interdisciplinary teams to develop individualized care plans, participates in clinical rounds, and provides drug information and pharmacotherapy recommendations.

Additionally, the role contributes to quality improvement initiatives, policy and protocol development, and clinical projects. The Clinical Pharmacist conducts medication use evaluations and research, and ensures compliance with institutional, state, and federal regulations to support safe, effective, and evidence-based medication use.

We are seeking a Per Diem Clinical Pharmacist to support ambulatory care services within a cardiology practice affiliated with Overlook Medical Center in Summit, NJ. The Clinical Pharmacist will play a key role in chronic disease management for cardiology patients at high risk of hospital readmission, working collaboratively with cardiologists, practice staff, and inpatient clinical pharmacists to optimize medication therapy and support seamless transitions of care.

Ambulatory

Care Clinical Pharmacist - Cardiology
  • Conduct comprehensive medication history and medication reconciliation reviews for cardiology patients, with a focus on those at high risk of readmission (eg heart failure)
  • Perform medication optimization, including assessment of medication access and affordability, and recommend appropriate alternatives or assistance programs when barriers are identified.
  • Provide patient education on medication regimens, adherence strategies, and self-management of chronic cardiovascular conditions.
  • Establish medication management goals of therapy in collaboration with patients and the care team, and monitor progress toward those goals.
  • Address patient-related medication questions from physicians and practice staff, serving as a drug information resource for the cardiology practice.
  • Collaborate with inpatient clinical pharmacists to ensure continuity of care across care settings, including post-discharge follow-up and transitions of care support.
  • Participate in and contribute to quality and process improvement projects related to ambulatory cardiology medication management and readmission reduction.
  • Document clinical interventions and patient encounters accurately and in a timely manner in the electronic health record.
Qualifications

Education/

Experience:

  • Graduation from an accredited School of Pharmacy with PharmD required
  • Current active registration and licensure with the State of New Jersey Board of Pharmacy required
  • Previous hospital pharmacy experience (2+ years) preferred.
  • PGY1 or PGY2 pharmacy residency training OR 2 years of clinical pharmacy experience in the assigned area
