Medical Records Analyst - Part Time, Shift, Mother Baby, Atlantic Health Medical Center

JOB FUNCTION:

Job Function: 01. Creation of Birth Certificate
- Gathers necessary data for completion of Birth Certificate and Fetal Death Certificate forms. Accurately enters all information needed for the birth certificate into the VERI. Obtains parental review of all information that will be included on the Birth Certificate. Obtains signatures as per DOH guidelines. Ensuring strict confidentiality at all times.

Job Function: 02. Educates/Assists
- Educates married parents on the process of their child's birth certificate and social security number. Educates unmarried parents to legally establish paternity of their child. Is able to communicate the process of submitting the Certificate of Parentage to the DOH and the time frame in which to expect documents to be ready. Provides parents with necessary documents i.e. proof of birth for insurance companies.

Job Function: 03. Maintain Data Integrity
- Prints basic reports for purpose of assuring integrity of data base and to assess and organize workload:
Daily Delivery Log, Admit Checks, DC Mom, DC Baby. Consults medical record and/or clinical resource (RN/MD) to verify data in the VERI. Makes needed corrections. Prints, scans, and emails when alteration of electronic record is necessary for the maintenance of data integrity.

Job Function: 04. Record Keeping/Birth Certificate Office
- Verifies the data entry of all births and fetal deaths into the VERI. Maintains a log of all births by date of birth, date of release to Morristown Town Hall. Maintains on-site files of all birth certificate/fetal death applications by ensuring placement of all documents with baby's medical record as well as any imperative documents in files within the office for future access.

Job Function: 05. Timely Completion of the Birth Certificate
- Assures physician certification of each live birth for the Birth Certificate. Completes all birth certificates (parents have proofread and signed) no later than the day before the mother is discharged. Communicates the reason for any exception to this standard to colleague following next day.

Job Function: 06. Releases
- Release of Birth Certificate within 5 days of the birth. Release of all other release points once data integrity has been reviewed.

Job Function: 07. Unresolved
- Maintains a low volume of unresolved records on the VERI queue. Reviews unresolved fields each day to ensure records are not withheld from release points.

Job Function: 08. VERI Admin Maintenance
- Education/Resource/Troubleshooting. Assists L&D and Mother Baby staff when VERI problems occur. Trouble shoots basic VERI computer problems or assists in obtaining help from ISS when necessary. Attends assigned number of staff meetings for the year.

Required:

1. High School Diploma required
2. Vital Statistic experience preferred
3. Preferred Veri and DOH experience
