Discharge Planner - Per Diem

Overview

The Discharge Planner – RN is responsible for managing each patient's plan of care, monitoring resource utilization, and coordinating discharge plans across a range of settings including Sub‑Acute units.

• Oversee the discharge process under the direction of the interdisciplinary team.
• Coordinate interdisciplinary care, ensuring patient needs and outcomes are met.
• Provide social assistance and file necessary discharge paperwork.
• Conduct initial assessments and develop individualized discharge plans.
• Assist patients in connecting with after‑care resources and community services.
• Maintain accurate documentation and comply with regulatory standards.

• Registered Nurse (RN) with state licensure.
• Minimum of a 2‑year degree or equivalent experience in discharge planning or social services.
• Strong organizational, communication, and problem‑solving skills.
• Ability to work collaboratively with an interdisciplinary team.
