Position: Social Worker (MSW) - Full Time - 8AM-4PM M-WF - Atlantic Health Overlook Medical Center, NJ

Job Description

Provides therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated, and uninterrupted care in the hospital and to ensure continuity of care in the community. Monday – Friday – 8:30 AM – 4:30 PM; weekend rotation required.

Responsibilities
  • Assesses the patient’s psychosocial situation in relation to their medical needs and plans appropriate interventions and treatment goals to improve patient and family functioning.
  • Interviews patients, family members, and/or significant others identified through case finding or referrals to assess needs for social work and/or interventions related to social determinants of health.
  • Participates in multidisciplinary rounds and provides input on the psycho/social context.
  • Acts as a resource and advocate on behalf of the patient and family for discussions involving advanced care planning.
  • Provides psychosocial support to patients and families during adjustment to illness and life changes.
  • Provides preventive, protective, or supportive services to patients who are in or threatened by situations detrimental to their well‑being.
  • Collaborates with the health‑care team regarding in‑hospital needs and post‑hospital care.
  • Maintains current knowledge of available psycho‑social service programs and provides relevant information to patients, families, staff, and community organizations.
  • Develops and maintains collegial relationships with community organizations.
  • Coordinates a viable psychosocial discharge planning program for the assigned service area to identify patient needs and plan for appropriate after‑care services.
  • Ensures continuity of care through comprehensive and appropriate discharge planning with patients, families, and significant others.
  • Documents data from psycho‑social assessments, individual contacts, family interventions, and discharge planning in accordance with department policy, procedures, and regulatory requirements.
  • Develops a social service network with city, state, and other governmental services to assist patients in obtaining such services as food stamps, financial supports, and transitional housing.
  • Evaluates psycho‑social services program objectives and results and participates in program planning and improvement in accordance with the mission of the hospital and department.
  • Maintains confidentiality in accordance with hospital and departmental policy and professional standards; may accompany patients to obtain benefits, entitlements, or other services.
  • Other duties as assigned.
Qualifications
  • Graduate of an accredited school of social work (MSW) required.
  • Previous medical hospital social work experience required.
  • NJ LSW or LCSW license required.
Competencies
  • Excellent communication, negotiation, and conflict resolution skills.
  • Knowledge of trends and advances in clinical practice and health‑care informatics, and new developments in technology.
  • Knowledge of relevant state regulatory standards related to social work.
  • Experience with rapid‑cycle change or clinical performance improvement.
