, serving Senior Living Communities across all levels of care. With a growing senior population, we understand the importance of being present throughout the geriatric continuum, promoting rehabilitation, prevention, and wellness to facilitate positive outcomes and fiscal success.

As a Physical Therapist serving our Veterans and Gold Star Families
, you will be responsible for providing quality direct patient care, including screening, evaluation, development of initial and ongoing treatment plans, discharge planning, and case management.

• Assess residents to determine care needs
• Optimize residents' functionality and skills
• Complete all necessary documentation via Net Health
• Attend required meetings and in-services, including but not limited to family meetings, care plans, staffing, and utilization reviews
• In conjunction with the DOR, develop programs appropriate to the needs of facility residents across the continuum of care
• Remain compliant with infection control and environmental protocols while following OBRA Resident Rights Guidelines
• If requested, provide consultation and clinical supervision to PTAs
• Maintain state license and stay current on issues related to physical therapy (company-paid)
• Responsible for appropriate scheduling of residents
• Facilitate and complete family and facility staff in-services and education
• Assist the facility in meeting standards necessary to maintain state and federal certification
• Complete tasks deemed appropriate and assigned by the Director of Rehabilitation
• Serve Veterans and Gold Star Families with pride and commitment

• Excellent communication skills, both written and verbal
• Ability to maintain professionalism in all situations
• Effective time management and the ability to manage a caseload
• Physical demands: required to perform a variety of duties, some of which fall into the moderate to heavy physical demand category

• Degree in Physical Therapy
• State licensure to practice in South Carolina
• Newly licensed Physical Therapists are welcome—we can support your career needs
