Sandhills Medical Foundation, Inc. is seeking a motivated, organized, and patient-focused Continuous Improvement Navigator to join our Continuous Improvement this role, you will help connect patients with the preventive and follow-up care they need while working alongside providers and clinical teams to improve patient outcomes and quality performance.
What You'll Do Identify and contact patients who are due or overdue for preventive screenings, chronic disease follow-up, annual wellness visits, and other services.
Assist patients with scheduling appointments and navigating healthcare services.
Maintain and work patient care-gap lists and help re-engage patients who have fallen out of care.
Identify barriers to care and connect patients with the appropriate resources or team members.
Review patient records to validate quality documentation and identify care or documentation gaps.
Prepare provider-specific care-gap reports and support provider huddles.
Assist with chart reviews, quality audits, data validation, and tracking.
Support quality initiatives related to UDS, PCMH, HEDIS, value-based care, and HRSA requirements.
Participate in quality improvement projects, including PDSA activities and initiatives focused on access, continuity of care, and patient engagement.
Accurately document outreach and maintain patient confidentiality in accordance with HIPAA requirements.
What We're Looking For Required High school diploma or equivalent.
Strong communication, organization, and customer service skills.
Ability to work independently and manage multiple priorities.
Strong attention to detail and ability to work with healthcare data.
Basic proficiency with Microsoft Office.
Ability to travel between Sandhills Medical Foundation locations as needed.
Preferred1–3 years of healthcare experience.
Experience in an FQHC, primary care, population health, care coordination, quality improvement, or patient outreach.
Experience working with an Electronic Health Record (EHR).Familiarity with healthcare quality measures, preventive care, or chronic disease management.
