Nurse Practitioner

Position Overview

Altais is a healthcare technology company focused on improving the experience for physicians and patients. This Nurse Practitioner role supports primary care services by delivering comprehensive assessments and documentation while maintaining a compassionate, patient-centered approach.

• Obtain history and perform physical examination.
• Complete review of medicines and related instructions.
• Review home safety and psychosocial needs.
• Fulfill documentation needs for STAR and HCC measures.
• Obtain and document all pertinent information in patient charts and case‑management notes on the same day as the visit, including medications, end‑of‑life preferences and drivers of preventable hospital admissions.
• Instruct patients, caregivers and families to call the NP prior to accessing medical care unless a life‑threatening emergency occurs.
• Maintain patient safety and confidentiality; apply universal precautions in all patient care.
• Prescribe or order procedures, treatments and medications within scope of practice.
• Develop a high level of trust and intimacy with patients and families.

• CA Licensed Nurse Practitioner
• 1+ year(s) of experience in primary care.

Base salary: $135,000 – $155,000 per year.

Work is conducted in an office environment (part‑time or full‑time). The role requires desk‑based work, with a predominantly sedentary activity level during most of the day.
