Registered Nurse Case Manager

We are seeking a Registered Nurse to service our patients throughout the Bluffton area. The Nurse Case Manager is a critical member of the patient care team, conducting patient visits, facilitating appropriate provider interactions, evaluating patients, and initiating telemedicine visits within the clinic setting. The role is responsible for providing quality healthcare in compliance with all applicable laws and policies within the scope of practice.

Day-to-day duties include performing visits in the Okatie clinic and traveling to patients’ homes or other locations. Must have reliable transportation and a valid driver’s license with an insured vehicle. The position is full‑time, salaried, with 8‑hour shifts Monday‑Friday (8:00–5:00).

Essential Responsibilities
  • Facilitate receiving all medical records from the patient’s primary provider and specialists.
  • Review medical records.
  • Complete consents with patients.
  • Enroll patients in Care Management if they meet eligibility criteria.
  • Initiate a Care Management Plan if the patient is eligible.
  • Capture diagnoses at highest specificity and ensure they are accepted.
  • Complete AWVs for provider review.
  • Conduct cognitive impairment screenings.
  • Conduct Social Determinants of Health (SDoH) assessments or screenings.
  • Prepare ACPs for review with the patient by the provider.
  • Assess eligibility for home health, hospice, palliative care, or consults with specialty divisions.
  • Evaluate eligibility for RPM devices, resources, or tools to improve patient quality of life.
  • Communicate and coordinate care.
  • Reconcile prescribed and OTC medications, vitamins, supplements, herbal remedies, and other treatments.
  • Assess need for adaptive equipment and DME.
  • Assess safety of environment.
  • Evaluate acute conditions or follow‑up from previous visit.
  • Accurately document and log Care Management activities; keep plans up to date.
  • Coordinate with the health care team, providers, therapists, home health or hospice representatives, and other stakeholders.
  • Facilitate visits with appropriate provider or entity.
Qualifications
  • Licensed nurse; RN preferred; license must be in good standing with appropriate board issuer.
  • Minimum of three (3) years of clinical experience preferred.
  • Experience in community settings preferred.
  • Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
  • Strong written and verbal skills.
  • Basic computer knowledge.
  • Demonstrated leadership and teamwork abilities.
  • Interpersonal and communication skills under all conditions.
  • Capable of fostering a cooperative work environment.
  • Team player able to manage multiple responsibilities with sound judgment.
  • Willing and able to work flexible hours and travel between offices, facilities, etc.
  • Licensed driver with an insured, reliable automobile.
