Well Care Health is seeking a passionate Home Health Licensed Practical Nurse to join our growing team and provide exceptional care in a supportive and rewarding environment.
Works under the supervision of the registered nurse. The licensed practical nurse assists in utilizing the nursing process (assessing, planning, implementing, evaluating) in Home Health according to policies and procedures.
Key Responsibilities- Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.
- Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
- Educate patients and families on health management, medications, and self-care techniques.
- Collaborate with the healthcare team to ensure the highest quality care for each patient.
- Assist the registered nurse with the implementation of the plan of care for patients in all developmental stages throughout the life span.
- Provide nursing care to patients in all stages of life based on experience and competency evaluation.

Job Duties
- Education:
Graduate of an accredited or approved school of practical nursing. - Licensure /
Certifications:
Current license to practice practical nursing in the State of North Carolina; CPR certification required. - Experience:
Minimum one year practical nursing experience required; home health experience preferred. - Essential Technical / Motor
Skills:
Hand/eye coordination for injections, computer use, patient care equipment manipulation, proper patient transfer and guarding; must be able to communicate and be literate in English. - Interpersonal
Skills:
Ability to develop positive interactions with patients, families, physicians, and staff; effectively care for the patients. - Essential
Physical Requirements:
Ability to transfer and/or maneuver objects weighing at least 50 pounds; frequent pushing, moving, lifting, positioning, bathing, ambulating; occasional reaching overhead, stair climbing, fine motor manipulation. - Essential Mental Abilities:
Must assess a patient’s condition, formulate a plan of care, select appropriate interventions, evaluate response to care, explain/teach patients; higher-level mental faculties; memory; prioritize duties; learn new skills and techniques. - Essential Sensory Requirements:
Visual assessment; hearing to auscultate lung, bowel sounds; hear alarms; effective communication. - Exposure to Hazards:
Noise, blood borne pathogens, body fluids, needle puncture wounds; exposure to dangerous animals and traffic hazards; personal safety threats; temperature changes and weather extremes.
