SUMMARY

The Migrant Education Program (MEP) Nurse Case Manager will provide one‑on‑one and group nursing case management services to address the combined health, social, and attendance needs of eligible migrant students in Columbia, Finley, Prescott, and Richland School Districts. The role provides health‑related education and resources to migrant students and families, MEP and district staff.

ESSENTIAL FUNCTIONS
  • Collaborate with MEP, school staff, and community partners
    • Identify students who may benefit from services
    • Assess students to identify health conditions
    • Describe how health concerns impact student engagement and success
    • Determine if the student would benefit from case management support
    • Prioritize students to include in the case load
  • Manage a case load of 40–60 students
    • Conduct an assessment to identify student and family strengths and goals
    • Include student and family to identify specific interventions to achieve goals
    • Manage referrals to support goals
    • Assist student and family to understand and manage interventions and recommendations from different referrals
    • Coordinate with school staff in development of student plans (emergency care plans, individualized healthcare plans, 504 plans, Individualized education plans, etc.)
    • Monitor plan effectiveness and make changes as needed
  • Collaborate with MEP to
    • Identify community strengths and concerns
    • Set community goals
    • Develop interventions to help achieve goals
    • Identify areas of opportunity to serve migrant students
  • Contribute to community events
  • Develop and facilitate education presentations in English and Spanish
    • Coordinate with community providers to provide support and presentations for community events
    • Understand migrant community and student issues that affect the success of migrant students
  • Document assessment, goals, interventions, and outcomes of work with students, families, community partners, and MEP staff
  • Support inclusion and advocate for the health rights of students and the migrant community
  • Provide consultation services to school personnel regarding health issues, e.g., communicable disease control
  • Serve as a resource for health education to students, families, and MEP program staff
OTHER JOB FUNCTIONS

Perform related duties as assigned.

MINIMUM QUALIFICATIONS
  • Current active Washington State Registered Nurse License
  • Current valid Driver's License
  • WSP/FBI background clearance required
  • Bilingual and biliterate in English and Spanish
PREFERRED QUALIFICATIONS AND SKILLS
  • Bachelor’s degree from an accredited nursing program
  • Two (2) years of experience practicing as a registered nurse
  • Demonstrate experience in one or more of the following areas
    • School nursing
    • Acute and/or chronic care in pediatrics
    • An independent practice setting (home care, hospice)
    • Computer skills to communicate and collect data
REQUIRES KNOWLEDGE OF
  • Professional nursing principles, standards of care, and state/federal obligations
  • Current health issues affecting rural and urban communities and public schools
  • Individualized Health Care and Emergency Care plans, Individualized Education Programs, 504 plans, and principles of school health programs
  • Child and adolescent health – physical, emotional, and mental health concepts
  • Regional school health resources and community partners
  • Professional interaction with diverse populations
  • Complex nursing, health, and education regulations
  • Equity promotion across all work and interactions
  • Develop rapport with students and parents while maintaining professional boundaries
  • Effective communication in English and Spanish, both spoken and written
  • Handling sensitive situations with tact, objectivity, and confidentiality
  • Utilization of health information software, technology, and standard office equipment
  • Independent work with large numbers of staff to promote the migrant health services program
  • Consistency and punctuality
  • Flexible hours and home visits to migrant families as needed
  • Provide first aid or CPR when needed
