The Community – Sumter, NC

Home to the University of Southern Carolina and Shaw Air Force Base, Sumter has come a long way since its days as a southern plantation settlement. Today, the centrally-located South Carolina city is known not only for its antebellum charm but its potential for economic growth, culture and leisure. Find a variety of fun things to do within Sumter's approximately 32 square miles.

During a sunny day, choose from a variety of outdoor activities. Rise early to enjoy a bicycle ride along Cypress Trail or a relaxing walk in Dillon Park. Before the morning is over, play a quick game of tennis at the Palmetto Tennis Center, swim at the city pool or splash in the sprays at Croswell Park. Since the city also manages 22 other parks and multiple trails, visitors will find an appropriate place to enjoy a picnic, play ball or throw a Frisbee before calling it a day.

However, no trip to Sumter would be complete without a visit to Swan Lake and Iris Gardens. The paths around Swan Lake cut through the only public park in the nation that houses all eight swan species. The lovely gardens feature 25 species of colorful, Japanese irises and offer a unique Braille Trail for the visually impaired.

Shaw Air Force Base

Located about 40 miles east of Columbia, South Carolina, Shaw Air Force Base ranks as one of the oldest installations in the Air Force and is the home of the 9th Air Force Headquarters and the 20th Fighter Wing and is the largest combat F-16 fighting wing. The Atlantic Ocean is 2 hours away with miles of beach available for vacation and recreational activities.

To the east, Myrtle Beach and Charleston await you, with wonderful seasonal activities and great southern food. Go North to enjoy cool mountain breezes or go white water rafting.

Summary

Provides case management, care coordination and discharge/disposition planning for inpatient and outpatient care settings. Assists service members and/or veterans, family members and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment;

addictions and other health problems. Coordinates care with multiple providers across all levels and sites of care. Addresses psychosocial as well as medical needs of patients and their families/caregivers through participation in multidisciplinary patient care management practice. Evaluates care and outcomes to ensure the most appropriate and effective care and services.

Requirements

(duties include but are not limited to)

  • Assessment
    - Proactively identifies and evaluates patients and families for case management from a variety of sources such as through discharge/disposition planning, referrals, the Medical Evaluation Board (MEB) process, the healthcare system, employers and facility staff. Conducts systematic, on-going, thorough collection of patient's physical, emotional, psychological, social and medical status and information to include direct patient contact and other relevant sources such as professional and non-professional caregivers, medical records, family/caregiver interviews.

    Conduct applied research and clinical investigations in clinical/behavioral health psychology.
  • Planning
    - Develops an appropriate patient-specific plan of care to include short and long term goals, objectives and actions. Coordinates, collaborates, and obtains approval of the plan among the patient, family/caregiver, primary provider and other members of the healthcare team.
  • Implementation
    - Guides the patient and family/care giver through the healthcare system, maximizing use of resources. Coordinates and executes the plan of care, optimizing access to appropriate services. Ensures necessary referrals are ordered by the appropriate discipline and coordinated. Serves as an advocate for, and ensures education is provided to, the patient and family/caregiver as required.
