Position Summary

This RN Navigator provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. The role collaborates with multidisciplinary teams, facilitates timely screening, diagnosis, treatment, and supports patients, families, and caregivers in overcoming barriers to quality medical and psychosocial care.

Core Functions
  • Assesses the patient’s plan of care and develops, implements, monitors and documents utilization of resources and progress through their care, facilitating options and services to meet health care needs.
  • Evaluates medical necessity and appropriateness of care, optimizing patient outcomes; serves as a liaison for referring physicians and assists with scheduling initial tests and consultations.
  • Assists the patient/family through diagnostic services, treatment and care; coordinates disease care using an interdisciplinary holistic approach and makes referrals and consultations in coordination with physicians and providers.
  • Ensures that patients and families understand diagnostic processes, care strategies, and recommended actions, providing appropriate information in coordination with physicians and health care providers; responds to patient requests for information regarding disease processes, expected side effects and community resources.
  • Evaluates patient’s functional abilities and limitations; determines if intervention is needed; establishes functional, measurable, patient‑related treatment goals and implements a plan of care to achieve them; collaborates with patient and family on goal setting and initiates discharge planning; recommends additions or modifications to referring orders.
  • Supports patients and families during difficult decision‑making periods; assists in coordination of end‑of‑life care and provides emotional support.
  • Supports process improvement activities for patient populations to achieve optimal clinical, financial, operational, and satisfaction outcomes; participates in staff development to maintain standards of practice and ensure highest quality of care.
  • Establishes and promotes collaborative relationships with physicians, payers, and other health care team members; collects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.
  • Works independently, determining how to best accomplish functions within established procedures; consults with supervisor on unusual situations; works facility‑based with no budgetary responsibility. Internal and external customers include staff, physicians, patients, families, other health care institutions, referring physicians, community providers, agencies, payers, provider networks, and regulatory agencies.
Minimum Qualifications
  • Bachelor's degree in nursing or related field.
  • Current RN license in the state of practice.
  • BLS certification.
  • At least 5 years clinical experience.
  • Excellent organizational skills.
  • Clinical knowledge regarding specialty care services, and care coordination of services, including legal and financial aspects of diagnostic services and health services in a specialty area.
  • Effective communication, writing, and time‑management skills.
  • Proficiency in word processing and database software applications.
  • Ability to teach clinical and non‑clinical personnel regarding care and diagnostic services.
  • Good understanding of process improvement.
Preferred Qualifications
  • Current certification in specialty area.
  • Prior case management experience.
  • Additional related education and/or experience.
