Key Responsibilities- Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs; review patient history and physical with patient/family; review diagnostics and laboratory data; complete assessments and reassessments per policy.
- Formulate a plan of care including discharge planning; initiate a care plan based on patient-specific needs; collaborate with the multidisciplinary team; revise the plan as patient needs change.
- Implement the plan of care through direct patient care, coordination, delegation and supervision of the health care team; ensure compliance with policies, standards of care and regulatory requirements; communicate status of patients to other caregivers.
- Evaluate patient and family response to care and teaching; adjust care plans as needed; monitor progress toward goals and outcomes.
- Document assessment, planning, implementation, and evaluation in the patient record; ensure timely and accurate documentation of data, interventions, and patient responses.
- Participate in evidence-based practice, unit or facility leadership activities, education, and mentoring; support growth of others through precepting when appropriate.
- Contribute to high-quality patient outcomes and engage in activities that promote safe, equitable care; participate in professional organizations as appropriate.
- In some roles, supervise staff and workflow within the department; interact with staff, physicians, patients, families and external contacts to coordinate care.

- Shift: 4 10-hour shifts, 6:30am-5:00pm

Yes, holidays, weekends and overnights with a 30-minute response time to facility - Differentials:
Weekend differential $3/hour; night differential 18% when applicable;
On-Call $5/hour; double pay for Callback

Summary
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and professional nursing practice standards. The nurse is accountable for the quality of nursing services delivered by self or others under their direction and uses specialized knowledge and clinical judgment to plan and provide care appropriate to the patient’s age and developmental status.
