Registered Nurse, Home Care

Responsibilities

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs
• Review patient history, physical examinations, diagnostics and laboratory data
• Report abnormal results to physicians and appropriate caregivers
• Formulate, initiate, review and revise individualized plans of care and discharge plans
• Provide direct patient care and coordinate, delegate and supervise health care team activities
• Provide care according to physician orders, nursing plans, policies, standards and regulatory requirements
• Evaluate patient and family responses to care and teaching
• Document assessment, planning, implementation, evaluation and physician orders in patient records
• Participate in research, shared leadership, education, orientation, precepting and mentoring
• Contribute to nursing quality improvement and patient outcomes
• Interact with staff, physicians, patients, families, community providers, agencies and clergy
• Synthesize and prioritize data to support patients and families during changes in health status

• Current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice
• BLS certification required
• Additional certification or continuing education may be required based on area of practice
• Must maintain clinical performance competencies through annual validations
• Ability to operate a personal vehicle in a Home Health setting
• Valid driver's license
• Active and maintained auto insurance coverage
• Arizona Fingerprint Clearance Card required at time of hire and throughout employment for Banner Home Care AZ employees
• Experience in the clinical area is desired
• Bachelor's degree preferred
• Professional certification preferred
• Additional related education and/or experience preferred

Demonstrates expertise in patient assessment, care planning, and coordination of health care team activities while ensuring compliance with regulatory requirements. Proficient in evaluating patient responses and contributing to quality improvement initiatives.
