POSITION SUMMARY  This position provides managerial and clinical support to patients and staff. Uses depth and breadth of clinical management knowledge to ensure the provision of high quality, safe, and cost-effective patient care. Demonstrates care coordination expertise and oversees the leadership of clinical, financial, and personnel management of the Care Coordination department. This position supervises employees and participates in selection, orientation, counseling, evaluation and staff scheduling.

Maintains clinical, leadership and post-acute care services knowledge and competency to evaluate outcomes related to delivery of Care Coordination services.
CORE FUNCTIONS  
1. Oversees Care Coordination personnel as assigned. Provides and/or facilitates Care Coordination and/or other related activities and serves as a resource to other staff members. Promotes utilization management through interdisciplinary patient care and activation of the discharge plan per the care delivery model.
2. Directs personnel actions including recruiting, new hire actions, interviewing and selection of new staff, salary determinations, training, and personnel evaluations.
