As an Advanced Practice Provider - NP, you deliver high-quality, patient-centered care. You serve as a vital member of our healthcare team, directly impacting patient outcomes and well-being. This role empowers you to utilize your advanced clinical expertise to assess, diagnose, and treat diverse patient populations, contributing significantly to our mission of health and healing. You build strong patient relationships, providing education and support that fosters trust and promotes health.
Job Duties and Responsibilities- Assesses patient health status through comprehensive histories and physical examinations.
- Diagnoses acute and chronic health conditions, formulating differential diagnoses based on clinical findings.
- Develops and implements individualized treatment plans, integrating evidence-based practices and patient preferences.
- Manages patient medications, including prescribing, monitoring, and adjusting dosages as needed.
- Conducts thorough physical examinations, evaluating all body systems to identify health issues.
- Documents patient encounters accurately and efficiently within the Electronic Medical Records (EMR) system.
- Educates patients and their families on health conditions, treatment options, and preventive care strategies.
- Interprets diagnostic test results, integrating findings into the overall patient care plan.
- Applies advanced pharmacological knowledge to ensure safe and effective medication management.
- Exercises sound clinical judgment in complex patient care situations, making critical decisions independently and collaboratively.
Education
- Graduates from an accredited Master's or Doctoral Nurse Practitioner program.
- Holds a current, unrestricted Nurse Practitioner license in the state of practice.
- Possesses current national certification as a Nurse Practitioner.
- Demonstrates at least one year of clinical experience as a practicing Nurse Practitioner.
- Shows experience in an outpatient or inpatient setting, managing a diverse patient panel.
- Possesses expert knowledge in Patient Assessment and Diagnosis.
- Excels in Treatment Planning and Medication Management.
- Performs comprehensive Physical Examinations with precision.
- Proficiently navigates and utilizes Electronic Medical Records (EMR) systems.
- Provides effective Patient Education with clarity and empathy.
- Interprets complex Diagnostic Tests accurately.
- Applies advanced principles of Pharmacology in practice.
- Exhibits superior Clinical Judgment and critical thinking skills.
- Holds additional specialty certifications relevant to the practice area.
- Demonstrates experience with quality improvement initiatives in a clinical setting.
- Possesses bilingual communication skills.
- Requires the ability to stand, walk, and move frequently throughout the workday.
- Involves occasional lifting, pushing, or pulling of up to 25 pounds.
- Works in a dynamic clinical environment with potential exposure to infectious diseases and various patient conditions.
- Performs tasks requiring manual dexterity and visual acuity.
